Specialist Support Coordination

Specialist Support Coordination is a higher level of coordination for people with more complex needs or higher risk, where supports may involve multiple systems (e.g., health, housing, justice, child protection) and can break down without intensive oversight. A Specialist Support Coordinator helps stabilise situations, reduce risk, and coordinate services so your supports are safe, consistent, and working toward your goals.

How We Help You

Community Participation

Put reliable supports in place and helps manage risks and barriers (like housing instability, safety concerns, or service gaps) so you can engage more consistently.

Employment

Coordinate complex needs (health, psychosocial supports, behaviour support, routines) to stabilise day-to-day life and create a realistic pathway toward work.

Education

Help organise multiple supports and adjustments, problem-solve disruptions, and keep the right services aligned so study is safer, more consistent, and sustainable.
